Client Preference Questionnaire

PURPOSE: The purpose of this questionnaire is to obtain information from the client so to customize the service provided. Your answers also provide me with your expectations of my service and of the results.

As this type of questionnaire is quite new and I would greatly appreciate your feedback with respect to the questionnaire as well as any aspect of my service.
